Youth Requirements – One-to-one Mentoring:

Youth must:

  • Be between the ages of 12 (or 11 turning 12 before the end of the year) – 16 to apply.
  • Youth will graduate the program upon turning 18.
  • Be able to communicate with their mentor.
  • Be able to listen to and follow instructions.
  • Be willing to learn and develop safe and responsible hunting ethics.
  • Be willing to learn and develop safe and responsible firearm handling habits.

For youth who would not otherwise have an opportunity to learn how to hunt.

The intake process consists of:

  • Completion of the youth and family application
  • Parent and child(ren) interview with SDYHA director
  • Completion of all required releases and paperwork

CONSENT / UNDERSTANDING

By signing below, I give permission:

  1. For my child to participate in South Dakota Youth Hunting Adventures
  2. For the volunteer matched with my child, who has been screened and approved by
  3. SDYHA, to transport my child to activities and events
  4. For the school to provide social and academic information about my child to SDYHA
  5. To have my child participate in and intake interview conducted by SDYHA staff and
  6. complete questionnaires throughout their time in the program that contain questions
  7. about their match, school, home life, and personal interests
  8. To have my child talk to SDYHA staff about personal safety
  9. For SDYHA staff to provide contact information for me and my child to the volunteer

TALENT/MEDIA RELEASE

I hereby consent to the use, sale, reproduction, transfer, or conveyance, of any and all photographs, images, likenesses, and voice or video recording of me in all forms (with or without the use of my name by South Dakota Youth Hunting Adventures or by any designee of the organization). I hereby consent to the use of all forms of media including advertising, trade, display, editorial, art, exhibition, and organization communications (with or without my photography and/or name). I hereby give this consent and release to South Dakota Youth Hunting Adventures, their nominees and designees from liability for any violation of any personal or proprietary right and agree to hold South Dakota Youth Hunting Adventures harmless from all liability from the use, sale, reproduction, transfer, or conveyance of the aforementioned items.

Please read the following carefully before signing this application:

I understand that the program is not obligated to match my child with a volunteer and that as part of the intake process I will be asked to provide additional information through an in-person interview. I understand that the information I provide in the enrollment process will be kept confidential, unless disclosure in required by law and with exceptions noted. I understand that incidents of child abuse or neglect, past or present, must be reported to proper authorities. I understand that certain relevant information about my child will be discussed with the volunteer who is a prospective match.

I do hereby release the organization and its employees, agents, members, volunteers and all other persons on its behalf from any and all liability for any damage or injury which such child might sustain while participating in said program and activities, including but not limited to any liability to any right of action that may occur to such child directly, or to me as their parent/guardian. I understand that this information may be shared with the school or with partnership agencies when applicable.

If my child is matched with a mentor, I agree to support my child’s match by reviewing the program and safety information given to me by South Dakota Youth Hunting Adventures, communicating with SDYHA staff as outlined, and immediately reporting any concerns I might have to SDYHA staff.
